As patients and their care partners navigate cancer care, they may encounter issues, questions and/or obstacles along the way. Although there may be solutions to help address some of these challenges, many people don’t find out about them when they need them most, which is why we created a problem solver called Health Match!
Health Match is a matchmaking platform that was co-designed to provide a lifeline to patients and their families by connecting them to solutions based on the issues they need help with. We partnered with patients and care partners to identify the main challenges they face, from the time they experience symptoms through long-term thrivership. We then mapped the tools, services and resources from credible organizations to address these obstacles.
