The “Roadmap to Wellness” provides the basics for people diagnosed with cancer and their care partners. The cancer planner is a three-ring, compact binder with ten chapters focused on helping you navigate next steps. It also includes an accordion file to help keep you organized. The “Roadmap to Wellness” was co-designed with 101 patients and their care partners, as well as healthcare professionals, patient educators, illustrators, information designers, and health literacy experts. It includes motivational illustrations, patient and care partner stories, infographics and templates to help you keep track of your health and better manage and coordinate your cancer care.

My world changed when I received
that shocking diagnosis. I was confused,

scared, and felt incredibly ill-equipped to answer
questions that would change my life and body, forever.
That same day, my surgeon gave me a CANCER101 (C101)
Planner. I started taking notes, getting second opinions,
collecting business cards, and it all began to fill the pages of my
